Sorry to hear about Milli. She is lucky to have such a good mom. They are so much like kids. You turn your back for one second and they get into something. Please keep us posted. You are both in my prayers. She is such a cute pup I can see why you fell in love.
 
my personal vote will go for home feeding. Checking your pup blood levels and urine is the only way to monitor if the diet works for you. You have more leeway to adjust things with home cooking than with any kibble.
Also I would try and find holistic vet- herbal remedies are great for cleansing kidneys and a lot could be done to improve general immunity which your pup will need to overcome this problem. Good news that your pup is young, there was single episode of damage which was addressed early- good indication that things might reverse itself pretty soon.
You might want to monitor water intake and general output pretty closely for while as too much of drinking and too dilute of the urine will present its own problems.

Have gone back to home feeding. No crystals in urine, no infection. Just need to make sure phosphorus levels are stable. More calcium to offset?
Just bought the Whole Dog Journals' set of books which had good info about feeding (they prefer BARF raw diet).

The Calcium to Phophorus ratio should be between 1.1 -1.4ish to 1 and no more than 2:1. So if you are lower phosphous you need to lower Ca too in that ratio. The body can't use the extra Ca if the P is too low. Too much Calcium will causes CA deposits and bone abnormalites in growing pups. Normal animal carcasses are 1.1:1 if you feed whole prey for example. Meat needs bone and eggs need shells to make a balnace diet for example.
